This notice applies to California residents who apply for employment, contractor, clinical, administrative, or other work opportunities with Care Talk Health. We provide this notice under the California Consumer Privacy Act, as amended by the California Privacy Rights Act, together referred to as the “CCPA.”
When you apply for a position with us, we may collect and use personal information about you to evaluate your application, communicate with you, conduct interviews, verify your qualifications and credentials, perform background, exclusion, sanctions, licensure, reference, and eligibility checks where permitted by law, complete hiring and onboarding activities, comply with legal and regulatory obligations, maintain security, and protect our rights.
Depending on the role and stage of the application process, we may collect the following categories of personal information:
Identifiers and contact information, such as your name, mailing address, email address, telephone number, professional profile information, government identification information where required, and similar information.
Professional, employment, and application information, such as your resume or CV, work history, references, credentials, professional licenses, certifications, NPI number, DEA registration where applicable, board certifications, specialty information, clinical privileges, malpractice or disciplinary history where applicable, interview notes, assessment results, and other information you provide during the application process.
Education information, such as degrees, schools attended, transcripts, certifications, training history, and continuing education information.
Internet, electronic network, and device information, such as information generated when you use our careers site, application portal, scheduling tools, or interview platforms, including IP address, device identifiers, browser information, log data, and similar technical information.
Audio, visual, or electronic information, such as video interview recordings, voicemail messages, call recordings, or other communications, where used in the recruiting process and permitted by law.
Protected classification information, such as race, ethnicity, gender, veteran status, disability status, or other demographic information, only where voluntarily provided or collected for equal employment opportunity, accommodation, compliance, or reporting purposes.
Sensitive personal information, such as Social Security number, driver’s license, state identification, passport, professional license identifiers, citizenship or immigration information, account login credentials for applicant systems, precise geolocation if enabled by your device or our systems, health or medical information related to accommodations, occupational health, vaccination, immunization, or fitness-for-duty requirements where applicable, and background check information where permitted by law.
We use sensitive personal information only for recruiting, hiring, credentialing, compliance, security, accommodation, eligibility, and other permitted employment-related purposes. We do not use sensitive personal information to infer characteristics about applicants except as permitted by law.
We may disclose applicant personal information to service providers, contractors, professional advisors, background check providers, credentialing and licensure verification providers, exclusion and sanctions screening providers, recruiting platforms, applicant tracking systems, interview and scheduling platforms, cloud hosting and IT vendors, affiliates, regulators, law enforcement, courts, government agencies, payers, credentialing bodies, or other parties where necessary for recruiting, hiring, credentialing, compliance, legal, security, or business purposes.
We do not sell applicant personal information or share applicant personal information for cross-context behavioral advertising. If our practices change, or if our recruiting website uses advertising or tracking technologies that constitute “sharing” under the CCPA, we will update this notice and provide any required opt-out rights.
We retain applicant personal information for as long as reasonably necessary for recruiting, hiring, credentialing, compliance, audit, legal, security, dispute-resolution, and recordkeeping purposes. For applicants who are not hired, we generally retain applicant information for [insert retention period, e.g., four years] after the final hiring decision or last application activity, unless a longer period is required or permitted by law. If you are hired, information collected during the application process may become part of your workforce record and will be retained under our workforce record retention practices.
